In the complex maze of American health care, affordability is the elephant in the room. Health care costs are skyrocketing, and it’s not just the uninsured folks who are sweating bullets. The average person now spends about $1,514 out-of-pocket, a staggering jump from just $115 in 1970, even when adjusting for inflation. Health insurance now covers a hefty 73% of total health spending, a significant increase from 27% decades ago. But hold on—this doesn’t mean the worries have vanished. The reality is that rising premiums and deductibles are leaving many feeling pinched.

In 2023, per-person health spending rose by 7%, and guess what? That’s just the beginning. Experts predict an average growth rate of 5% annually from now until 2032. The folks in worse health or those scraping by on lower incomes? They feel the heat more than anyone else. But affordability issues aren’t picky; they’re affecting millions across the U.S. Insured adults are losing sleep over their monthly premiums—40% are seriously concerned about affording them. And let’s not even get started on deductibles; 62% of insured adults worry they won’t be able to meet those costs before their insurance even kicks in.

For those under 65, the situation gets even murkier. Uninsured adults report a staggering 82% difficulty affording care, while those with coverage show a “mere” 44%. Many people with employer-sponsored insurance rate their plans as “fair” or even “poor” with regard to premiums and out-of-pocket costs. Almost four in ten adults under 65 are sweating bullets over monthly premiums. That’s a lot of anxiety over something that should be a safety net, not a noose.

As of March 2025, about 60% of people under 65 had employer-sponsored insurance. But eligibility has dipped slightly, with many eligible workers opting out due to cost—30% cite expense as a deal-breaker. Of those with employer-sponsored coverage, a good chunk is reliant on family members for their insurance, highlighting just how complicated this maze truly is. In fact, 80.4% of adult workers under 65 worked for an employer offering ESI to at least some employees in March 2025.

Healthcare is now the top household expense, surpassing food, rent, and utilities. It’s almost comical—except no one’s laughing. A family of four, just to keep the insurance wheels turning, contributed over $6,296 in premiums in 2023, on top of $3,564 in out-of-pocket expenses. That’s nearly one in every five dollars spent in the entire U.S. economy. Adding further pressure, ACA marketplace premiums are projected to rise by an average of 7% in 2025, with some insurers requesting increases exceeding 10%. And as private insurance costs are predicted to rise further, households are bracing for more financial strain.
